There is no doubt that aesthetic ideals are changing. Gone are the days of excessive treatments and over corrections. Longevity and a healthy, natural appearance are in vogue, bringing regenerative aesthetics into the spotlight. One of the most experienced voices in this field is Dr. Bettina Kerbler.

As an internationally recognized Key Opinion Leader in biostimulators and regenerative aesthetics, Dr. Bettina Kerbler has a wealth of expertise in guiding patients through a variety of treatments. She is also the founder of the AVORYA clinic on the shores of Lake Zurich, Switzerland.

In our latest conversation, we delved into the trend toward effortless, natural results and the evolving expectations of today's discerning patients. Take a moment to read what she had to say about the latest on regenerative aesthetics.

How is regenerative aesthetics reshaping beauty trends in today’s clinics?

The aesthetic field is constantly changing. It moves from one extreme position, to somewhere in between, and then in the complete opposite direction. A few years ago, it was trendy to inject large volumes of filler. Having no lines, and no facial movement, was seen as attractive in some countries—it was almost considered a status symbol.

Now the trend is heading in the other direction. Patients want results to look completely natural and they want long-lasting results. Hyaluronic acid has lost its appeal, Botox is being questioned, and gray hair and no makeup are trendy. To achieve natural-looking results, it’s essential to understand the aging process in the different layers of the face and treat them holistically, so a combination of treatments is key.

[Editors note: Signs of aging occur in several tissues. The skin becomes thinner and starts to sag, muscles lose a bit of definition, fat pads decrease and descend toward the jawline and there will also be some bone resorption and remodeling that change the underlying support.]

How important are biostimulators in your clinical practice?

Biostimulators activate the body's own collagen production and are therefore in high demand at the moment, whether it’s PLLA, polynucleotides, or exosomes. After negative campaigning by some plastic surgeons against hyaluronic acid fillers, fewer patients want to have filler injections.

Also, there is a broader trend toward a natural appearance and a healthy look. During consultations, patients are increasingly asking about skin quality improvement, whether through biostimulators or laser devices. For example, I used to inject around 1,000 syringes per year, but this year only 92. Instead, I have injected more than 700 vials of JULÄINE.

Who benefits most from biostimulators in regenerative aesthetics?

My target group is changing at the moment—even patients in their 20s and 30s are concerned with issues like poor skin quality, enlarged pores, and skin laxity, and are seeking treatments to improve these conditions.

The older my patients are, the more I need to explain that all facial layers must be addressed—sagging, for instance, is a multifactorial issue. That said, biostimulators are always my first choice in the treatment process. For example, the better the skin quality, the less filler I ultimately need.

One of my best results with JULÄINE was actually on a 75-year-old woman, though it’s worth noting she had been receiving regular CO₂ laser treatments for 10 years and still had good skin quality. Tightening the deeper layers of the skin creates a beautiful lifting effect.

Does regenerative aesthetics truly meet today’s patient expectations?

Patients naturally want to look young, but some expect us to wave a magic wand and make all their problem areas disappear, which isn’t possible. Even a facelift can’t address everything, because not all layers of the face are treated—the skin quality hasn’t changed, nor has the muscle tone, or the lack of volume. Biostimulators are an ingenious solution, but like facelifts, they can’t solve everything. The most important thing for me is to give adequate information and advice so that patients understand what to expect.

What advice would you give patients considering regenerative treatments?

Choose an aesthetic professional who is well-trained and wants to see you in person rather than only offering consultations over video. They should take the time to assess your skin’s thickness and quality and have access to a broad range of treatments. This way, they can select the most appropriate option for you, rather than being limited to whatever is available.
